使用者，您好。
為有效建立儀器履歷資料庫，凡發表之碩博士論文或期刊著作中，有使用國防醫學院貴重暨共同儀器中心服務所列之項目儀器者，於成果發表時，並不要求將服務提供者列為共同作者，但須於文章之致謝欄中載明：
『感謝國防醫學院貴重暨共同儀器中心提供ｘｘｘ技術服務。』
且提供論文封面及載明頁面影本各一份，供本中心留存。

Or you want to be more specific:

Please also provide the title page and the acknowledge page of the thesis or the paper to the instrument center.
